Copays, Coinsurance, Deductibles

+ Patients are responsible for verifying eligibility with their network insurance providers regarding applicable out-of-network benefits, policy restrictions or reimbursement procedures. For those insurances that we are out of network with, a superbill for submission and possible reimbursement can be provided upon request.

+ The patient’s portion of an insurance claim is due at the time of service. A valid credit card should be kept on file with Phoenix Mental Health & Wellness at all times.

Self-Pay Costs

New Patients

  • Initial Evaluation

    Approximately 75-90 minutes - $350

Existing Patients

  • Routine Follow-up or Medication Management

    (30 minutes) - $150
    Can be in-person or via telehealth

  • Transfer Appointment - Follow Up For Medication Management & Continuation of Care

    (45 minutes) - $150
    Can be in-person or via telehealth

Cancellation Policy

Appointments cancelled or rescheduled need a 24-hour notice prior to start time of appointment to avoid a no-show fee, which is half of the self-pay cost of the designated appointment. 

Please note that insurance does not cover no show fees and it will be charged to the credit card on file.

As a small private practice, we are not set up to be a crisis or emergency service and all patients are seen on a scheduled basis.
